Telehealth Informed Consent

Telehealth means receiving evaluation and care from a licensed clinician remotely, using video, telephone, or secure messaging instead of an in-person visit. Your clinician may review your questionnaire, medical history, laboratory results, and anything you share during the visit.

Benefits include convenient access to clinicians without travel, shorter waits, and easier follow-up. Limitations include the inability to perform a hands-on physical examination; in some cases your clinician may determine that telehealth is not appropriate and refer you to in-person care, laboratory testing, or another provider. Technology can fail; if a visit is interrupted, we will reconnect or reschedule promptly.

• Consent to telehealth is voluntary; you may withdraw it at any time
• Withholding or withdrawing consent will not affect your right to future care
• You may ask questions about your care, alternatives, and costs at any time
• The privacy protections described in our Privacy Policy apply to telehealth visits

• Provide accurate, complete health information, including current medications
• Be located in the state you told us at the time of your visit
• Take part in visits from a private, safe location (not while driving)
• Follow up with in-person or emergency care when your clinician advises it

Telehealth is not for emergencies. If you experience a medical emergency, call 911 or go to the nearest emergency room. If you are in crisis, call or text 988.

Visits are conducted through systems with safeguards appropriate for health information, and your records are handled as described in our Privacy Policy. We do not record visits. Please do not record visits without the consent of everyone present.

Whether any medication is appropriate is decided solely by your clinician after evaluation. Prescriptions, when issued, are sent to an appropriately licensed pharmacy. No specific medication or outcome is guaranteed, and your clinician will explain expected benefits, risks, alternatives, and monitoring before you decide.
